- Provide at least 24 hours notice for appointment cancellations

4. Appointments and Cancellations
Appointments are scheduled based on availability and medical necessity. We reserve the right to reschedule appointments due to emergency situations or unforeseen circumstances.
Cancellation Policy:
- Please provide at least 24 hours notice for appointment cancellations
- Late cancellations (less than 24 hours) may result in cancellation fees
- No-show appointments may be charged the full consultation fee
- Repeated no-shows may result in termination of patient relationship
5. Payment Terms
Payment is due at the time of service unless other arrangements have been made.
- We accept cash, major credit cards, and most insurance plans
- Patients are responsible for understanding their insurance coverage
- Any balance not covered by insurance is the patient's responsibility
- Past due accounts may be subject to collection activities
- A service charge may be applied to returned checks

Our liability is limited to the direct cost of services provided.
